Bacteria has evolved, it's structures has adapted to antibiotics (medication), a lot of reasons have identified as to why antibiotic resistance has taken place.
Underdosage is most why bacteria resist from being killed by most medicine (antibiotics).

Using contaminated syringes alters a drugs efficacy, thus will not be able to effectively destroy bacteria.
